A tool for reflection
The change predictor tool identifies a series of factors
that may be involved in successful management of change
through partnership. The tool is based on the lessons learned
from the implementation of the Sheffield Health Action Zone
programme to tackle health inequalities.
You can use this tool in relation to a programme or initiative
that you are currently planning, or it can be used to reflect
on something that has worked or has not worked for you in
the past. More importantly, these factors may throw light
on why a change programme can be implemented successfully,
but with little impact.
